ABSTRACT

OBJECTIVE@#To compare the effect of @*METHODS@#A total of 74 patients with RIF of thin endometrium type undergoing freeze-thaw embryo transfer were randomly divided into an observation group (37 cases) and a control group (37 cases). The patients in the control group were treated with freeze-thaw embryo transfer in hormone replacement cycle, and the estradiol valerate tablets were taken orally from the fifth day of menstruation, 2 mg per day. On the basis of the control group, the observation group was additionally treated with @*RESULTS@#The clinical pregnancy rate was 37.8% (14/37) in the observation group, which was higher than 16.2% (6/37) in the control group (@*CONCLUSION@#On the basis of conventional medication,

ABSTRACT

BACKGROUND@#Delivery room resuscitation assists preterm infants, especially extremely preterm infants (EPI) and extremely low birth weight infants (ELBWI), in breathing support, while it potentially exerts a negative impact on the lungs and outcomes of preterm infants. This study aimed to assess delivery room resuscitation and discharge outcomes of EPI and ELBWI in China.@*METHODS@#The clinical data of EPI (gestational age [GA] <28 weeks) and ELBWI (birth weight [BW] <1000 g), admitted within 72 h of birth in 33 neonatal intensive care units from five provinces and cities in North China between 2017 and 2018, were analyzed. The primary outcomes were delivery room resuscitation and risk factors for delivery room intubation (DRI). The secondary outcomes were survival rates, incidence of bronchopulmonary dysplasia (BPD), and risk factors for BPD.@*RESULTS@#A cohort of 952 preterm infants were enrolled. The incidence of DRI, chest compressions, and administration of epinephrine was 55.9% (532/952), 12.5% (119/952), and 7.0% (67/952), respectively. Multivariate analysis revealed that the risk factors for DRI were GA <28 weeks (odds ratio [OR], 3.147; 95% confidence interval [CI], 2.082-4.755), BW <1000 g (OR, 2.240; 95% CI, 1.606-3.125), and antepartum infection (OR, 1.429; 95% CI, 1.044-1.956). The survival rate was 65.9% (627/952) and was dependent on GA. The rate of BPD was 29.3% (181/627). Multivariate analysis showed that the risk factors for BPD were male (OR, 1.603; 95% CI, 1.061-2.424), DRI (OR, 2.094; 95% CI, 1.328-3.303), respiratory distress syndrome exposed to ≥2 doses of pulmonary surfactants (PS; OR, 2.700; 95% CI, 1.679-4.343), and mechanical ventilation ≥7 days (OR, 4.358; 95% CI, 2.777-6.837). However, a larger BW (OR, 0.998; 95% CI, 0.996-0.999), antenatal steroid (OR, 0.577; 95% CI, 0.379-0.880), and PS use in the delivery room (OR, 0.273; 95% CI, 0.160-0.467) were preventive factors for BPD (all P < 0.05).@*CONCLUSION@#Improving delivery room resuscitation and management of respiratory complications are imperative during early management of the health of EPI and ELBWI.

ABSTRACT

· AIM:To observe the surgical results of large astigmatism with small incision lenticule extraction (SMILE) for 2a.· METHODS:Totally 33 eyes of 17 consecutive patients were enrolled in this retrospective study,aged from 18 to 36 years old,which included 24 eyes of 12 patients with astigmatism within-3.50D to-4.00D and 9 eyes of 5 patients within-4.25D to-5.00D,7 eyes of 4 patients with sphere within 0 to +1.00D and 26 eyes of 13 patients within-1.00D to 0.Intended cap diameters was 7.3-7.5mm,lenticule diameter was 6.6-6.8mm,the thickness of cap was 120μm,the surgical incision was 2mm.We observed the results after 1d,1wk,1,3,6mo,1 and 2a.Preoperative best corrected visual acuity,spherical equivalent refraction,postoperative uncorrected visual acuity,SimK equivalent value and SimK value was observed.· RESULTS:Compared with preoperation,uncorrected visual acuity showed significant difference statistically at 1d postoperatively (P< 0.05).Compared with 1wk postoperatively,uncorrected visual acuity showed no significant difference statistically at postoperative 1,3,6mo,1 and 2a(P>0.05).Uncorrected visual acuity did not increase or decrease more than two lines.The target diopter was +0.25D.Compared with 1d postoperatively,the spherical equivalent refraction and residual astigmatism showed no significant difference statistically at each postoperative period (P> 0.05).postoperative SimK equivalent value and SimK value difference showed no significant difference statistically compared with each postoperative period (P>0.05).· CONCLUSION:It is both safe,effective,predictable and stable to correct large astigmatism with SMILE by long-term observation,postoperative 2a.The corneal morphology is good and worthy of clinical application.

ABSTRACT

AIM:To investigate the protective effect of nerve growth factor combined with Ginkgo biloba extract on retinal ischemia-reperfusion (RIR) injury in rabbits with experimental high intraocular pressure.METHODS:Establishment of rabbit glaucoma ischemia reperfusion model.Twenty-four New Zealand white rabbits were randomly divided into three groups:nerve growth factor group, Ginkgo biloba extract group and combination group.Respectively, in the continuous administration of 1, 7, 14d.We observed the morphological changes of the tissues of the retina.The levels of superoxide dismutase(SOD), nitric oxide(NO) and malondialdehyde(MDA) in retinal tissue were measured.RESULTS:Respectively, first, in the continuous administration of 1, 7, 14d, the contents of MDA and NO in Ginkgo biloba extract group and nerve growth group were higher than that in combination group (P<0.05).Secondly, the SOD content of Ginkgo biloba extract group and nerve growth group were lower than that of combination group at each time point (P<0.05).At each time point, the number of HE staining of retinal ganglion cells (RGCs) showed that the loss of RGCs in the combination group was significantly lower than that in the other groups, and the ganglion cell count showed that the Ginkgo biloba extract group and the neuronal growth group were lower (P<0.05).CONCLUSION:Nerve growth factor combined with Ginkgo biloba extract has better protective effect on retinal ischemia-reperfusion injury.The mechanism may be related to the decrease of free radicals and increase the activity of SOD in retinal tissue.

ABSTRACT

AIM:To observe the efficacy and safety of 1g/L bromfenac sodium hydrate ophthalmic solution in the partial substitution of glucocorticoid after laser subepithelial keratomileusis (LASEK).METHODS: Totally 104 cases (208 eyes) were received LASEK, which were selected and divided into study group and control group.The study group were adopted 1g/L bromfenac sodium hydrate ophthalmic solution combined with tobramycin dexamethasone eye drops and fluorometholone eye drops, the control group were adopted tobramycin dexamethasone eye drops and fluorometholone eye drops.The changes of visual acuity and intraocular pressure of two groups were recorded before and after surgery, and the score of painness and the occurrence of haze were observed after surgery.RESULTS: At postoperative 1, 2, 3d, respectively, the pain score of the study group were 1.70±0.35, 1.25±0.34, 0.82±0.32, the pain score of the control group were 2.30±0.43, 1.68±0.44, 1.12±0.33, the differences were significant (P0.05), and the differences were not significant(P>0.05).At preoperative and postoperative 1, 2wk, 1 and 3mo of surgery, respectively, the intraocular pressure of study group were 17.33±1.58, 7.54±1.28, 7.23±1.58, 7.26±1.47, and 7.30±1.36 mmHg;the intraocular pressure of control group were 17.53±1.43,7.57±1.32,7.73±1.55,7.80±1.38,7.86±1.43 mmHg,the differences were not significant between before and at 2wk after surgery(P>0.05), the differences were significant between the two groups at 2wk, 1mo and 3mo (P0.05).CONCLUSION: It is safe and effective that 1g/L bromfenac sodium hydrate ophthalmic solution in the partial substitution of glucocorticoid after laser subepithelial keratomileusis.The patient has a lower intraocular pressure, has similar therapeutic effect as glucocorticoid in vision and antiinflammatory.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the factors that affect the clinical pregnancy rate after intrauterine insemination (IUI).</p><p><b>METHODS</b>We retrospectively analyzed the clinical data of 611 IUI cycles and analyzed the relationship of the clinical pregnancy rate after IUI with the female age, infertility duration, type of infertility, cycle number, cycle protocol, thickness and type of endometrium, and semen parameters before processing.</p><p><b>RESULTS</b>The clinical pregnancy rate was significantly higher in the ovulation induction than in the natural ovulation cycles (23.03% vs 11.03%, P < 0.01), but lower in the 4th cycle and above than in the 1st, 2nd, and 3rd cycles (2.04% vs 21.30%, 18.13%, and 12.67%, P < 0.01). Marked decreases were found in the clinical pregnancy rate in the females aged ≥ 40 years and the cases with pre-processing total progressively motile sperm count (TPMSC) < 10 x 10(6), progressive motile sperm < 20%, or morphologically normal sperm < 2% (all P < 0.05). Logistic regression analysis revealed the cycle protocol, cycle number, and percentage of progressively motile sperm as three predictive variables affecting the clinical pregnancy rate after IUI (P < 0.05).</p><p><b>CONCLUSION</b>The cycle protocol, cycle number, percentage of progressively motile sperm, female age, TPMSC, and sperm morphology are the main factors affecting the clinical pregnancy rate following IUI, while infertility duration, type of infertility, and thickness and type of endometrium exert little influence.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the mechanism of natural moxibustion on regulating immune imbalance in asthma rats.</p><p><b>METHODS</b>Seventy SD male rats were divided into a normal group, a placebo group, a dexamethasone group, a big-cake for long-course moxibustion group, a big-cake for short-course moxibustion group, a small-cake for long-course moxibustion group and a small-cake for short-course moxibustion group, ten rats in each one. The rat model of asthma was established by egg albumen sensitization and stimulation in all the groups except the normal group. The natural moxibustion was used in all moxibustion groups, in which big cake of 1 cmX 1 cm size was used in the big-cake groups and small cake of 0.5 cmX 0. 5 cm size was used in the small cake groups. According to relevant acupoints, the natural moxibustion was performed, 5 h per time, once a day. Four times of treatment was considered one course, and three courses were required in the long-course groups and one course was required in the short-course groups. Intraperitoneal injection of dexamethasone was applied in the dexamethasone group, which had the same course as long-course moxibustion group. After the treatment, changes of EOS in peripheral blood of asthma rats were observed; enzyme linked immunosorbent assay (ELISA) was applied to test the contents of IgE, IL-4 and IFN-gamma in the lung tissue; real-time Q-PCR method was adopted to measure the expression level of transcription factor T-bet and GATA binding protein 3 (GATA-3) in the lung tissue.</p><p><b>RESULTS</b>Compared with the normal group, the EOS in whole blood as well as IL-4 and IgE in plasma were all increased in the placebo group (all P< 0. 01), IFN-gamma in plasma was obviously decreased (P<0. 01); while the levels of EOS, IgE and IL-4 were significantly reduced (all P<0. 01), the content of IFN-gamma was increased (P<0. 01) in all moxibustion groups and dexametnasone group. Compared with the normal group, the expression of T-bet mRNA in the placebo group was significantly reduced (P<0. 01). Each treatment group could significantly increase the expression of T-bet mRNA and reduce that of GATA-3 mRNA (P<0. 01). Compared with the short-course moxibustion groups, the expression of T-bet mRNA was obviously increased in the long-course moxibustion group and dexamethasone group (both P<0.01), and that of GATA-3 mRNA was reduced (P<0.01). There was no significant difference between long-course moxibustion group and dexamethasone group (P> 0. 05), and also no significant difference could be seen between big-cake moxibustion group and small-cake moxibustion group (P>0. 05).</p><p><b>CONCLUSION</b>The natural moxibustion could obviously reduce airway inflammation in asthma rats. With time passing, the efficacy is enhanced, indicating evident timeliness, which has no apparent relationship with the size of moxibustion cake.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe the therapeutic effect and mechanism of new percutaneous absorption herbal patch for asthma of paracmasis, and to optimize the form of the patch.</p><p><b>METHODS</b>One hundred and twenty cases of paracmasis asthma were randomly divided into medicine patch group, medicinal vesiculation group and western medication group with 40 cases for each. The new percutaneous absorption herbal patch was applied on medicine patch group. Traditional medicinal herbs cake of the hospital were applied on medicinal vesiculation group. Feishu (BL 13), Fengmen (BL 12) and Dazhui (GV 14) were adopted for both groups. Each patch was applied for 6 hours and once every other day. Accuhaler was applied on the western medicine group with 2 inhalations a time and twice a day. Clinical symptom scores, number of attacks and asymtomatic days were observed right before, after and half a year after the treatment. Meanwhile, the expression level of IgE, IL-4, GATA-3 mRNA and T-bet mRNA were observed and compared before and after the treatment.</p><p><b>RESULTS</b>Clinical symptom scores of all the 3 groups were improved (all P < 0.01). The differences of the total effective rate, number of attacks and asymtomatic days of all the 3 groups are without statistical significance (all P > 0.05). However, the long-term therapeutic effect in half a year after the treatment showed that the total effective rate of the medicine patch group was 80.0% (32/40), and the medicinal vesiculation group was 70.0% (28/40). Both of the them were obviously higher than 47.5% (19/40) of the western medicine group (P < 0.01, P < 0.05). And the medicine patch group surpassed the other 2 groups in controlling the number of attacks and increasing the asymtomatic days (P < 0.05, P < 0.01). The level of IgE and IL-4 of all the 3 groups decreased sharply after the treatment (P < 0.05, P < 0.01). And there was no statistic significance in differences among groups (all P > 0.05). The level of GATA-3 mRNA was obviously decreased, while the level of T-bet mRNA was obviouly increased in the medicine patch and medicinal vesiculation groups (P < 0.05, P < 0.01). And the medicine patch group had obvious superiority on increasing the level of T-bet mRNA when compared with the medicinal vesiculation and western medicine groups (P < 0.01, P < 0.05).</p><p><b>CONCLUSION</b>It is concluded that the new percutaneous absorption herbal patch has exact effect on asthma. The treatment may reverse the imbalance condition of Th1/Th2 through regulation on cell factor and its specific transcription factors.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the clinical therapeutic effect of catgut implantation at acupoint on climacteric syndrome (CS) and its mechanism.</p><p><b>METHODS</b>Sixty cases were randomly divided into a catgut implantation group and a western medicine group, 30 cases in each group. The catgut implantation group was treated by catgut implantation at Shenshu (BL 23), Zigong (EX-CA 1), Sanyinjiao (SP 6). The western medicine group was treated by oral administration of Diethylstilbestrol tablet. Their therapeutic effects and changes of content of follicle-stimulating hormone (FSH), luteotrophic hormone (LH), estradiol (E2) and beta-endorphin (beta-EP) in serum were observed.</p><p><b>RESULTS</b>The total effective rate was 93.3% in the catgut implantation group and 70.0% in the western medicine group with a significant difference between the two groups (P < 0.05). After treatment, the beta-EP concentration in the catgutation implant group was significantly increased (P < 0.05), the FSH concentration was significantly reduced and E2 concentration was significantly increased as compared with those in the western medicine group (P < 0.05, P < 0.01).</p><p><b>CONCLUSION</b>Catgut implantation at acupoint has an obvious therapeutic effects on CS. It can significantly improve the ovaries function and has a favorable regulative action on the endogenous opioid peptides.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To search for the best therapy for recurrent herpes simplex.</p><p><b>METHODS</b>Ninety-six cases were randomly divided into an auricular point sticking plus moxibustion group (treatment group, n = 54), a medication group (control group, n = 42). The treatment group were treated with auricular sticking at points Spleen, Lung, Kidney, and others, and moxibustion was applied at Zusanli (ST 36), Fenglong (ST 40), and local blister; the control group were treated by oral administration and external application of aciclovir.</p><p><b>RESULTS</b>The cured rate and the total effective rate were 55.5% and 100.0% in the treatment group, and 9.5% and 61.9% in the control group, respectively, with a significant difference between the two groups (P<0.05).</p><p><b>CONCLUSION</b>The auricular point sticking plus moxibustion is a better therapy for recurrent herpes simplex, with a significantly better therapeutic effect than that of the medication.</p>
